|Subject:||Payroll Deductions Authorized for Fiscal Year 1998-1999|
Applications submitted by payroll vendors for continued deduction authorization for Fiscal Year 1998-1999 have been processed and approved. A list (VC-01) of authorized vendors is attached. A listing (VC-02) of vendors, authorized products, and assigned codes, as well as, the name, address, and phone number of the vendor coordinator is also attached for your reference.
Note: Several vendors have designated new coordinators for the 1998-1999 plan year.
After a detailed review of vendor applications, the following changes have been made to miscellaneous vendors/products for Fiscal Year 1998-1999. No new products were authorized for this plan year.
MSA - American Heritage is no longer offering the EIWL Int. Whole Life product (MSA NPz). Employees that currently have this product will continue to have it coded under NZ; however, no new policies can be written and set up for payroll deduction.
MSH - Protective Life Insurance has opted to discontinue participation in the cafeteria plan, because of OSUPs requirement that across the board rate increases for cafeteria vendors must be effective July 1. As a result, employees who have the cancer product (MSH PA/NA) will no longer have that deduction sheltered beginning with the first pay day in July. OSUP will automatically convert all MSH PA/NA deductions to MSH NM on June 16, 1998.
MSN - New York Life is no longer offering the Annuity product (MSN NRz). Mark Duffy of New York Life has informed us that there are no UPS employees with an annuity deduction.
MSYG - Effective July 1, 1998, State Employees Group Benefits (SEGBP) will allow all employees (cafe and non-cafe participants) to purchase SEGBP miscellaneous products. Therefore, OSUP will unrestrict the non-participating codes for catastrophic illness (MSYG NA) and Vision (MSYG NB) for the first payday in July.
Payroll deduction forms (SED-4s) will be revised to reflect the changes in assigned codes and product names. Forms for employee deductions effective July 1998, that do not reflect the products/codes shown on the current VC-02 for that vendor and/or forms that do not contain all required information, should be returned unprocessed to the vendor. New forms should indicate the employee's cafeteria participation status at the time the form is signed.
