The "AS OF DATE" in the upper right hand corner of the Misc Income and Fringe Benefits Taxable Report (BL1501-01) is incorrect. The date listed on the report is 07/13/97, but should be 06/15/97. The data contained on the report is correct (as of pay period ending 06/15/97). Please refer to Office of State Uniform Payroll Memorandum #98-01 for additional information about the Misc Income and Fringe Benefits Taxable Report.
