SUBJECT:		Adding Agency Telephone Numbers to ISIS Check Stubs

Due to the heavy volume of telephone calls OSRAP receives from the State's vendors requesting supporting documentation and/or information regarding ISIS checks, the ISIS check stubs will be changed to include agency contact telephone numbers. The agency telephone number will be input into what is currently the AFS AGC2 "Manager" field. The label of this field will be changed to "Phone #" at a later date. When an agency issues payment, its telephone number will print on the check stub preceding the payment line information. For example on the check stub preceding a payment by agency 326, the following information will print:

326 OFFICE OF PUBLIC HEALTH PHONE #(504) 568-1097-5096.

As shown above, you may include an extension number if it can be dialed directly. You may only provide one contact number per ISIS agency number. This information should be provided to us as soon as possible. If this information changes, you should notify OSRAP immediately.

For your convenience you may use a copy of this memorandum to supply us with the needed information. The information can be completed below.
