INSURANCE INFORMATION NOTICE NO. 98-1

SUBJECT: GLASS REPLACEMENT CLAIMS FOR STATE-OWNED VEHICLES

The policy provided by the Louisiana Self-Insurance Program has a $100 deductible for comprehensive coverage. When glass (windshield, door glass, etc.) on a State vehicle needs to be replaced, the State agency is responsible for the first $100 of the invoice. You should direct the glass vendors to bill your respective agency, not the Office of Risk Management (ORM). If a State agency desires to file an insurance claim with our office, they should complete form DA 2073 (Vehicle Glass Repair/Replacement Loss Notice) and submit it along with an original invoice from the glass vendor (not a work order or photocopy of the invoice). At that time, ORM will process the claim and issue a check directly to the glass vendor for the amount in excess of the deductible and in accordance with the provisions/conditions of the glass replacement contract issued by the Division of Administration - State Purchasing Office.
