REGISTRATION FORM
The PDF
fill-in CPTP Class Registration
Form form allows you to enter information on the displayed
form and then E-mail the form to your supervisor (or agency
CPTP Coordinator). Or, you can just print the completed form
and send it to the appropriate people for approval.
To view, complete, and print this form, you must have the
Adobe Acrobat Reader version 4 or higher, which is free.
To get a copy of the reader, click on the Get Acrobat Reader
button.
How
to fill in the form:
- Position
the cursor inside a form field. The I-beam cursor
allows you to type text. The pointing finger (or arrow)
allows you to select the checkboxes.
- After
entering text or selecting a checkbox, choose from the following:
- Press Tab to accept the field change and go to the next
field.
- Press Shift+Tab to accept the field change and go to the
previous field.
- Press Return or Enter to accept the field change and deselect
the current field.
Note: In a multiline text field, Return
goes to the next line; it does not accept the field change
nor deselect the current field.
- Keep
text within the visible boundaries on the form. Text
outside of the boundaries will not print.
- To
send the form to your supervisor (or agency CPTP Coordinator)
using E-mail, you must fill in your E-mail address and
your supervisor's (or agency CPTP Coordinator's) E-mail
address where indicated on the form.
- Once
finished, either click the "Send to Supervisor"
button to send the form through E-mail to your supervisor
(or agency CPTP Coordinator), OR print the form and
submit it for approval.
