Hospitalization Changes for Plan Year 1999-2000 |
The following hospitalization providers have been approved for Plan Year 1999-2000:
|
A - Group Benefits PPO |
D - Ochsner |
|
E - Group Benefits EPO |
V - United (Lafayette Area Only) |
An automated conversion will be processed for the employees affected for the following changes in hospitalization coverage. These system transactions will be done on Tuesday, May 18, 1999, before the Master File is available to users for updates and will appear on a separate On-Line Master Record Audit Report produced May 18, 1999.

Since Advantage Health is no longer an approved provider for this plan year, all employees with Advantage Health coverage codes will be converted to the corresponding Group Benefits PPO coverage codes - UPS Code A-- . |

(2) |
Since Group Benefits will no longer offer the Catastrophic Illness (CIE) rider, all employees with Group Benefits CIE coverage codes will be converted to the corresponding Group Benefits PPO coverage codes with no CIE. |

(3) |
Premium/deduction amounts for all employees will be converted to the new rates. At this time as part of Cafeteria maintenance, all employees with a "S" split HS or LI participating code will be converted to a "P" participating code. Refer to OSUP Memorandum #99-50 on cafeteria maintenance. |
Although these automated transactions are being done in UPS, agency payroll personnel are responsible for entering employee hospitalization changes which occur during open enrollment. If changes made automatically in UPS do not agree with the coverage the employee has elected on the Group Benefits enrollment or change form, agencies must enter those changes into the system. Some examples of this may include the following:

If an employee who currently has Group Benefits chooses Group Benefits EPO or United. (Note that employees who currently have Group Benefits coverage will be identified in UPS with coverage of Group Benefits PPO after the new codes are added to the system.) |

(b) |
If an employee who currently has Advantage chooses Group Benefits EPO or Ochsner. |
However, since premiums for basic coverage are the same for all providers, agencies have until pay period ending June 13,1999, to enter changes made during open enrollment as long as the coverage level does not change and coverage does not include a psych rider. All changes must be entered before the monthly reports/files are produced for Group Benefits on June 14, 1999.
New rates for the following valid UPS Hospitalization Codes for Plan Year 1999-2000 will be established in UPS for the first payroll in June:
