Payroll Deductions Authorized for Fiscal Year 1999-2000

Applications submitted by payroll vendors for continued deduction authorization for Fiscal Year 1999-2000 have been approved. A list of authorized vendors (VC-01) is attached. A list of vendors, authorized products, and assigned codes, as well as, the name, address, and phone number of the vendor coordinator (VC-02) is also attached for your reference.

Note: Several vendors have designated new coordinators for the 1999-2000 plan year.

After a detailed review of vendor applications, the following changes have been made to miscellaneous vendors/products for Fiscal Year 1999-2000:

Two new miscellaneous vendors were established in UPS for pay period beginning December 28, 1998, and continue to be authorized for plan year July 1999 - June 2000.

Prepaid Legal Services, Inc. is offering a legal service plan.

Transamerica Life Companies is offering a long-term care plan.

Effective pay period beginning June 14, 1999, the following changes will occur:

Mid-West National Life Insurance Company (MSM) will no longer be an authorized payroll vendor. Refer to OSUP Memorandum #99-38 for additional details.

Group Benefits (MSYG) has decided to no longer sponsor the catastrophic illness and vision products. Since the catastrophic illness product is also being sold by Colonial under the vendor code MSK, UPS will change the catastrophic illness deduction code on the employees' records from MSYG PA/NA to MSK PA/NA. For the vision product, UPS will stop the deductions for MSYG PB/NB on the employees' records.

System generated transactions for the MSM and MSYG changes will be created 6/15/99 for the first payroll in July. Refer to OSUP Memorandum #99-50 for additional information.
