Stop Payment Request Reports and Revised Forms |
Effective April 1, 1999, the stop payment reports, "Report of Incomplete Stop Pay Requests" and "Quarterly Stop Payment Requests", will no longer be issued. Due to these changes, OSUP will require the appropriate certificate (UPR/F162, UPR/F164) be attached with each stop payment request. A stop payment and void/reissue will not be processed until OSUP receives the appropriate certificate.
Attached find revised forms UPR/F161 Stop Payment Request and UPR/F164 Certificate of Lost Checks. These revised forms should be used when submitting your stop payment requests.
Please update Sections 2.07 and 5.01 of your Standard Accounting Procedures Manual. The revised forms will also be updated on the Internet in Section 5.01. If you should have any questions regarding the above changes, contact Laura Odom at (225) 342-0717.
